The hospital readmission rate for Congestive Heart Failure (CHF) in the 30 days following discharge is an alarming 24% nationally. Hospital readmissions are costly in terms of patient health and quality of life, hospital resources and payer reimbursement. To help solve this issue, several innovative hospitals have looked to the consumer and retail industries for methods of consumer understanding and influencing behavior change, including psychographic segmentation and digital engagement.

Patrick Dunn has served in the field of cardiac rehabilitation as an exercise physiologist, program director, and educator for over 30 years. He has worked extensively with both the American Heart Association and the American Diabetes Association on both the national and local levels. His research is in health literacy instructional strategies and has developed a model for developing health literacy skills based on qualitative, quantitative and mixed methods research.
