Solving CHF Readmissions Using Psychographics + Technology

VIDEO_ICON_WEBINAR_WHITE

Presented on Tuesday, Oct. 24, 2017

*Now Available On Demand*

Session Description

The hospital readmission rate for Congestive Heart Failure (CHF) in the 30 days following discharge is an alarming 24% nationally. Hospital readmissions are costly in terms of patient health and quality of life, hospital resources and payer reimbursement. To help solve this issue, several innovative hospitals have looked to the consumer and retail industries for methods of consumer understanding and influencing behavior change, including psychographic segmentation and digital engagement.

AHA_NoLIW_K+R

This webinar, co-hosted by the American Heart Association, addresses the opportunity for reducing hospital readmissions – as well as initial admissions – for cardiovascular conditions, and provide case studies demonstrating how the application of consumer methods can lead to significant results.

Learning Objectives

In this webinar, participants will learn:

  • The scope of the opportunity regarding hospital readmissions for CHF and other cardiovascular issues
  • What psychographic segmentation is and how it can supplement medical care
  • How to implement a program to reduce hospital readmissions using psychographics and automated patient engagement
